You will likely have many questions about the process of becoming a new patient here. We'll attempt to answer as many questions as we can in advance of your becoming a patient at our facility. Before being seen at the International Craniofacial Institute, you should ask your insurance company about whether or not you need a referral from your primary care physician. If you need a referral, we have included a physician referral form on this website for your convenience. In addition, talking to your insurance company about the care you'll receive here at the International Craniofacial Institute can be complicated, therefore we recommend reading the section on Insurance and Cost for answers to many of the issues you may encounter. If you're traveling a distance to receive your care here, we've provided information regarding housing and other services you may need while you're in this area. In addition, we have included a New Patient form, for your convenience, which can be completed online or downloaded to your computer then completed prior to your arrival.
